Football Registration Forms
You will need ALL of the forms listed below. Please print these out, fill them out completely, and bring them with you to a registration date.
Don't worry if a form won't download. We'll have extras at sign ups. You may need the latest version of Adobe Reader to access some forms.

In addition, you will need the following:

$75 deposit, or pay the entire fee of $175

Copy of a birth certificate

Current wallet-size photo (please NO bigger than wallet size)

Insurance or Medi-Cal policy number, if applicable

13 & 14 year olds must present proof of grade level to show eligibility for the 2008 season. A current year report card works best.
Teams fill up quickly!
Make sure you have all of the required documents and the deposit when you sign up. Please understand that we can not place your child on a team without all of the above information. Any registrations completed after July 1st, 2008 will result in a late fee assessed of $30. This includes registrations documents turned in after the July 1st deadline.
