You will need ALL of the forms listed below. Please print these out, fill them out completely, and bring them with you to a registration date.

In addition, you will need the following:

• Copy of a birth certificate
• Current wallet-size photo (please NO bigger than wallet size)
• Insurance or Medi-Cal policy number, if applicable
• 13 & 14 year olds must present proof of grade level to show eligibility for the 2008 season. A current year report card works best.
• $75 deposit (or pay the entire fee of $295)
